In the event of a fatal accident or where a major injury is suspected, the person in charge must notify the Practice Director.
All serious accidents, dangerous occurrences and/or near misses will be thoroughly investigated by the Practice Director. A report including, where appropriate, photographs and witness statements will be prepared.
All accidents and near misses will be reviewed and where necessary changes to working practices and procedures implemented.
